PLEASE BRING REQUIRED DOCUMENTATION

All participants must have purchased a tryout membership through the Puget Sound Region/USAV to attend tryouts.

All interested players must complete the following steps prior to participating in any form of tryouts, practice, or competition.

​

1. Please review the USA Volleyball Junior Player Age Definition Chart before registering for tryouts.

2. Fill out and submit the Online Registration.

3. Sign and date the online WAA/GTA Athletic Waiver.

4. Read, print out, and sign a hard copy of the USAV Medical Authorization Form and turn it in at Tryouts.

5. Read, print out, and sign a hard copy of the USAV/PSRV Concussion and Sudden Cardiac Arrest Form and turn it in at Tryouts.

Check-In: 9:30am

Tryouts: 10:00am - 12:00pm

 

U15-U16 Players 

Check-In: 11:30am

Tryouts: 12:00pm - 2:00pm
